Water pump shaft.
I've heard that the water pump shaft will break just from age or wear and send the fan into the radiator, no other defects, just worn and broke. Said it was a "common" problem. True?

True, happened to my friend's Bronco while we were driving around once. Fan shaft snapped and fan disintegrated and dug itself into the radiator. It was pretty awesome but sad for his car.
I think it's probably more likely to happen if you fan clutch starts locking up. May want to check that.

No fan clutch
The shaft doesn't break until a month or two of "GGGGRRRRRRRRR" noises from the bearing, which should give ample warning to all except those with loud stereos or cell phones stuck in their ears.
